Bariatric surgery is expensive procedure for rapid loss of the weight through any type of surgeries, such as;
- a laparoscopic adjustable gastric band or lap band,
- gastric bypass surgery (such as Roux en-Y [proximal], Roux en-Y [distal], and Loop Gastric bypass)
- vertical banded gastroplasty, and – gastric sleeve surgery.

Despite each type of surgeries may have some complications, such as infection, hemorrhage, hernia, bowel obstruction, venous throboembolism, gallstones, nutritional deficiencies, nerve complications, and even death, including expensive cost around $20,000 to $30,000, these bariatrics
surgery, effectively to treat health problems related to obesity and morbid obesity based on current guidelines related to body mass index requirement (adults with BMI of 40 kg/m2 or higher, BMI of 35 kg/m2 or higher for adults with additional risk factors, and BMI of 30 to 40 kg/m2 plus additional risk factors who fail treated with exercise and dietary intervention, including pharmacotherapy).

One of new suggestion related to health problem for the needS of bariatric surgery, especially with laparoscopic adjustable gastric banding (LAGB), was in obese patients with type 2 diabetes.

As the use of bariatric surgery to treat diabetes remain controversy in healthcare circles, as many critics question the wisdom of wielding a scalpel to solve a medical problem, especially when clinicians have more drugs at their disposal to deal with diabetes, while also recent previous study has raised the doubt of efficacy of LAGB, the International Diabetes Federation (IDF) who announced bariatric surgery is recommended for obese patient with type 2 diabetes, moreover with additional cardiovascular risk factors such as hypertension, hyperlipidemia, and history of heart attack, said that their recommendation is only one option and for specific patients, as they found some of types of diabetes are well treated by lap-banding early in the disease proses.

Therefore, the IDF calling for other randomized controlled trials to compare different bariatric procedure for diabetes themselves, as well as emerging non-surgical therapies.
